If you are ready to fill in your pre-consult form, you can find the details here. If you’ve formally booked my services and would like to fill out a Birth Plan, print and complete the plan listed in the Labour and Birth Guide used by BC Women’s Hospital. We will review and revise it together during your prenatal appointments or please, come with your own!
